Quick Quoter Terms and Conditions

Welcome to the Quick Quoter ("QQ") provided by Sure Plans LLC, d/b/a Sure Plans Managed Benefits. Before proceeding, you must read and agree to the following Terms and Conditions ("Terms"). By clicking "I AGREE" you acknowledge that you have read, understood, and agree to be bound by these Terms.

1. Nature of the Quote

• Quote is an Estimate: Your quote is based on the unverified information you have entered. It is not a guarantee of coverage or a binding offer to enroll in any Sure Plans Managed Benefits product or service.
• Contingent on Verification: The final quoted rate is contingent upon the verification and accuracy of all the information you provided. We may require supporting documentation to verify this information.
• Quote and Enrollment Timelines: This quoted price is valid for seven (7) days from the date it is generated. You must accept these Terms and Conditions within this 7-day period. Upon acceptance, you will have ten (10) days to complete your enrollment to secure the quoted price and coverage.
• No Obligation: Receiving or accepting this quote places you under no obligation to enroll in any program.
• Errors and Omissions: This digital quote is provided for informational convenience and is not warranted against equipment, digital, or human error.

2. Your Declarations and Responsibilities

By using the QQ, you represent and warrant that:

• Accuracy of Information: All information and answers you provide are accurate to the best of your knowledge and belief. You have provided an accurate email address and contact phone number.
• Decision-Making Authority: You are the legal decision-maker and payer for any policy that may result from this quote.
• Legal Eligibility: You and any dependents you wish to enroll are legally permitted to enroll in Major Medical, Life, Dental, Vision, and Retirement Plans within the United States as of today's date.
• Cooperation: You agree to provide all requested or required documents in a timely manner to facilitate the verification and enrollment process. Product details are for your use alone and are not transferable to any other person or entity.

3. Consent for Verification and Transactional Communications

• You agree that Sure Plans Managed Benefits can text and call you at your verified, provided number for each Quote Acceptance (e.g., via One-Time Password or OTP). Message and data rates may apply.

4. Consent for Marketing Communications

By clicking "Accept Quote" and providing your contact information, you:

• Provide express written consent for Sure Plans Managed Benefits to contact you via telephone call, text message and or email with offers or other messages regarding the products and services about which you have inquired.
• Acknowledge that we may use an automated dialing system, artificial or prerecorded voice, or other automated technology to contact you at the phone number provided.
• Understand that you may receive calls or messages even if your telephone number is currently on any internal, corporate, state, federal, or national Do-Not-Call (DNC) list.
• Understand that your consent to be contacted for marketing purposes is not a condition of purchasing any product or service.

5. Enrollment and Payment Terms

Should you choose to enroll after accepting the quote, you agree to adhere to these payment terms:

• Payment Due Date: Your initial program payment funds must be available for drafting from your provided, verified bank account within ten (10) days of your agreed-upon enrollment date. Credit cards are not accepted.
• Auto-Draft Requirement: Enrollment and funding requires your agreement to continuous monthly auto-drafts for premium payments.
• No-Increase Guarantee: A "No-Increase Guarantee" on your rate is conditional upon your continuous and successful monthly payments via auto-draft and your timely replies to administrative and membership updates and emails.

6. Privacy

Your privacy is important to us. By agreeing to these Terms, you also acknowledge that you have read our Privacy Policy and California Privacy Notice.
